Standard 10: Quality Improvement
You want your participants
to benefit from your DSMES service.
Knowing if they are benefitting
requires you to be able to
analyze outcomes and evaluate your
DSMES service's effectiveness.
Process, clinical, and behavioral outcomes
can all be measured.
Diverse criteria such as
number of referrals,
A1c levels,
and behavioral goal achievement
are just a sample of
the measures that can be used.
Having these data allows
the quality coordinator to determine
the effectiveness and impact of
DSMES services and to implement
continuous quality improvement.
Data collection and analysis should be
completed with each DSMES participant
to determine current
successful strategies
and to identify areas for improvement.
Using these data,
the DSMES quality coordinator
can objectively see how
the DSMES service is performing
and establish a plan
for improving the service.
